AZALEA TRAIL NURSING AND REHABILITATION CENTER

WOW. Wonderful. The Azalea Trail team members have recently had the annual state survey and now for three consecutive years, had zero deficiencies! Congratulations. They are also on Facebook having FUN activities and great food. I’m so proud of them.

My mom is so happy here. The attention she receives is exactly what she needs and wants. She loves the food. We are so thankful for Azalea Trail! With regards to the office management - I’m so pleased! We have a long term care policy that has been a nightmare to deal with. But Marilyn stays with it and takes care of every detail they demand. And she does it cheerfully! Thank you! 😊 I highly recommend Azalea Trail!

The staff have been wonderful with my mother and us. They truly make her feel as though Azalea Trails is her home. It is wonderful to go, any time of the day or night, and see the staff with positive and upbeat attitudes. This is a hard job and the staff truly have the heart and passion for their jobs and patients.
