My mother fell at this nursing home and suffered with a brain bleed for 9 days without being seen by a doctor. My mother, a native Spanish speaker but is bilingual, has been living here since 2023. For the first 2 years it was fine and we believed she was getting the care she needed. Then, in mid 2025 we noticed that her dementia symptoms had started to worsen and we asked the facility to review her medications to see if they could help. The facility told us they couldn’t do that since she was on hospice and hospice controlled her medical care. We proceeded to speak with hospice and the facility to urge them to work together to make the changes she needed. After 4 months of back and forth and no action being taken, my mother, in her confusion tried to elope from the facility. This facility had a code on the door that changes frequently and there was no way for her to know that code. She followed a vendor out of the facility, in her wheelchair, without them knowing. Thankfully a nurse saw her and brought her back in. The director of the facility, Sam Major, and the director of nursing asked us to come in and speak with them. We came to the facility within 30 mins of the call. We stood in the lobby waiting for them and didn’t see a single staff member near the front door for 20 minutes. Finally, we see Sam run by us barefoot in basketball shorts and a t shirt. He was also soaking wet. He proceeded to run into his office and close the door, and changed into corporate clothing. He explained that they were doing an employee appreciation event. This was unacceptable to us as we had been asked to come to the facility as soon as possible and then had to wait to speak to someone who was in that state. It was also unacceptable that no one was anywhere near the front door the whole time we waited to ensure the residents didn’t leave. At this point, Sam and director of nursing proceeded to tell us that they wanted to transfer my mother to a locked down facility. We refused due to the dementia and the confusion that would cause. We explained that we had been asking them and the hospice company to look at her medications and adjust them and showed them text messages going back months with no action. Finally, they told us that the hospice was getting in the way of her care and we should revoke hospice and let them manage her care. On November 5th, we revoked her hospice care at the urging of the facility. On November 8th we received a call from the facility stating that my mother had a bad fall that was unwitnessed. When we arrived, she had a terrible bruise on the right side of her head and she was more confused than usual and couldn’t speak in clear sentences, even in Spanish. Her speech was slurred and her eyes were glassy. We asked what medical attention they were giving her and they stated they did X rays and nothing was broken. Everyday we went to the facility and her condition continued to worsen to the point that she couldn’t feed herself or dress herself. More than that, she developed more bruises and a huge knot on her left cheek and breast area that could have only occurred with at least one other fall. Finally, on November 17th, we forced the facility to send her to the hospital. They said it wasn’t an emergency and didn’t call 911 but they called an ambulance transport. 7 hours later, they got her to the hospital where they did a CT scan that showed a brain bleed that needed emergency surgery. My mother has now been in the hospital for over 2 weeks. Monday, December 1st, Sam took all of her belongings, including her family photos, religious objects, toiletries, food and shoes and unceremoniously threw them all together in garbage bags. He treated her belongings as if she was not a human being whose life and belongings would even matter to her or to her family. My mother’s traumatic brain injury has robbed the family of the short amount of time we had left with her not because she has died but because she’s no longer in a state to remember us or communicate with us and her death has been accelerated due to this injury.

After being diagnosed with Small Cell Lung Cancer and going through several medical procedures at Baylor due to various complications. My mom spent 2-weeks in their rehabilitation program to rebuild her strength for her upcoming chemotherapy. And found everyone to be extremely friendly. There were a few issues, which we were happy to see management quickly resolved. My remaining items would be to upgrade, the meals and changes to the parking with more spots in the front lot.
