A great facility for your love ones. Awesome rehabilitation Staff is great! And one thing very important it’s clean !! I needed my mother’s bed fixed, maintenance came right away to and fixed it. If you need anything they’ll take care of it. Over all a great place! :)

I would not recommend Fort Worth Transitional Care Center, it does not even deserve the one star review. During my mother’s stay, I found her in diabetic shock, partially out of bed, seemingly unattended for hours. Staff response was slow and unprepared—I had to request a glucose check and then had to request for paramedics to be called. Equipment was in poor condition: broken bed rails, a wheelchair with only one working brake, and a promised mattress that was never replaced. Basic needs like emptying a portable toilet required repeated requests, and nurse call responses were delayed. Overall, the level of care was unacceptable. It was evident that the staff there were not happy to be there.

The new administrator Bradley Crow is an amazing leader. The staff are very good with the residents and listen to your concerns and care about their residents. Would recommend for anyone looking for a place that truly cares about their loved ones.

Carley's Analysis

Carley found families praised the quality of care and rehabilitation services, though persistent staffing shortages and facility cleanliness concerns were significant recurring issues.
