Legend Healthcare is hands down the best place for your loved one to come for skilled or long term care. My grandmother, great-grandmother and a close family friend all received excellent and compassionate care at Legend Healthcare!
This facility should not be allowed to operate. It does not even deserve one star. My grandfather endured severe mistreatment during his 10-day stay. He was degraded, spoken to with disrespect, and repeatedly neglected. He was admitted for in-house rehabilitation after a severe UTI and fall, with the hope of regaining strength and returning home. Instead, the neglect was so extreme that we were forced to transfer him to another facility for his safety. Staff went an entire week without bathing him, leading to painful bedsores and severe rashes which he never had a problem with before. He was isolated in his room, denied help getting out of bed except for one or two trips to the bathroom a day, and given only one hour of physical therapy Monday through Friday. His medications—including critical blood pressure and mental health prescriptions—were never administered on time, putting his well-being at serious risk. It breaks my heart that we ever placed him here. The care he received was not just inadequate—it was inhumane. I would never recommend this facility to anyone, especially for a loved one who cannot speak up for themselves.
If I could give this facility zero stars, I would. My 74-year-old grandfather, a Vietnam veteran, was admitted after a severe UTI and a fall, with the goal of completing short-term rehabilitation to regain strength and safely return home. He remained at Legend Healthcare and Rehabilitation in Paris, Texas, for 10 days, and the care he received was unacceptable. During his stay: • Isolation: He was confined to his room for the entire 10 days, except for one hour of physical therapy Monday through Friday. This level of isolation appears to be in violation of patient rights. • Lack of Dignity and Respect: He was frequently spoken to in a dismissive or condescending manner. Several nurses and aides told him he should “do things himself,” despite his admission being for inpatient rehab, not long-term nursing care. • Poor Hygiene Care: He received only two showers during his 10-day stay — one on day three and another six days later — despite being incontinent and having a suprapubic catheter that requires strict hygiene to prevent infection. • Medication Mismanagement: His medications were not given on time, including multiple blood pressure medications and mental health prescriptions that directly affect his daily functioning and stability. • Cold Meals and No Assistance: All three daily meals were served cold, and staff refused to cut his food so he could eat, despite his limited arm mobility and the fact that he has no teeth. • Unprofessional Conduct by Staff: • An employee named Laura told my grandfather he was “in the wrong place” and should “find somewhere else to go.” • An employee named Valerie told him he needed to “go back to Oklahoma.” • An employee named Amnestie refused to bring him coffee, refused to pick up his phone charger from the floor, and even raised her voice at him for asking for assistance. We have video documentation of staff speaking to him in an unprofessional and dismissive manner. The entire experience was degrading, unsafe, and delayed his recovery. Based on this experience, I would never send a family member — especially someone who cannot advocate for themselves — to this facility. Patients deserve compassion, dignity, and timely care, none of which were provided here.
