My grandfather has been at this facility since Tuesday, and the experience has been deeply concerning. We have not been able to speak with anyone who actually knows what is happening with his care. Staff cannot tell us what medications he’s receiving, when they’re given, or even what the process is for tracking and communicating patient updates. He has been left sitting in dried urine for far too long, and we cannot get consistent updates about his physical therapy sessions. Multiple staff members say they will follow up, but nothing gets done. His blood work was supposed to be checked this morning, and that never happened. The lack of urgency is alarming. My grandfather is not alert, not communicating, and not eating—and somehow no one here sees this as a problem. He looks sedated and completely out of it, as if he was given medication that was far too strong. There is no order, no communication, and no sense of responsibility for the people in their care. I would not trust this facility with my family member, it is clear they are overwhelmed, do not have good leadership and unable to provide the level of care they advertise. This was not only sad, but very concerning for the other patients especially those without family. I would strongly recommend choosing any other facility. Do NOT bring your loved one here. My grandfather is now being transported to Harris Hospital after a noticeable decline in his health over just three days under their care. This place is unsafe. Run! I will officially be filing a complaint and reviewing options to hold the staff accountable at all levels.

My mother has been here over a week and so far everyone has been attentive, helpful, and kind to her. Therapy has helped her get stronger daily and the food is decent. A few things could improve when it comes to their hygiene. Showers should be given as scheduled and this should be a priority for the patient. I get this place is busy and probably under staffed but please improve this. Patients are scheduled showers every other day so keep this organized & up to routine please and thank you. There have been a few hiccups but overall the care everyone shows for the my mother & patients is comforting. My mother has good things to say, it’s been a pleasant stay so far.

My mom has been admitted here 3 times now for rehab. She’s been lots of other places as well and this is the best by far! She loves her nurses and aids and most of the meals. (She’s limited by allergies. No fault of theirs.) Care Plan meetings are offered by phone but you can attend in person. My only negative was that therapy wasn’t present for her meeting and that’s what she was there for, but everything was handled well and all questions were answered fully and honestly. Be your own advocate or if it’s your loved one, be theirs! No one is a mind reader. Ask questions. Provide feedback. It really does help and they strive to be the best!

Carley's Analysis

Carley found serious communication gaps and inconsistent care responses were major concerns, though rehabilitation services and facility cleanliness were generally well-regarded.
