Incident report for 12/18/2025. Such a TERRIBLE Experience on this date. I have a wound on my bottom and when they “get me up” into a wheelchair, sometimes it is extremely painful, which it was on this date. A PT person by the name of Brian “had me” for exercise for 2 hours, but my pain prevented me from doing anything and the only relief would have been to get me back into bed so that I could lay flat to relieve the pain. Everyone knew it, including Brian, but he refused even though I demanded 3 times to be put back in bed to alleviate the pain, but he refused to do so, even though I was crying due to the pain. He would leave the room and not return for 10-15 minutes just leaving me in agony!!!. Brian was useless and did not understand that when a patient is in so much pain, and he knows what the solution is, that he should perform that task to bring comfort back to the patient, but NO, he could not be bothered. Finally a good hearted person by the name of Diana stepped in and got me back into bed, even though that was painful, she did it and laid me flat which subsided the pain. I can not tell You enough how grateful I was to get someone that understood about patient comfort. The people in “charge” should have to experience such pain and have the employee refuse to listen and will not perform what he knows is the solution. I would bet that such person would be fired. I am in a situation where I can not stand or walk, otherwise I would have been able to do something about it, but in this condition, I am at the mercy of the employee. 12/24/2025 - By Leon Shaffer *********** I believe that I must revise my posting above and state that the incident was a one time experience and does not reflect an accurate description of the facility. I must say that a better review is that the facility is a very good place, if You or one of Your loved ones needs to be. If You are looking for a place, I am pretty sure that You are concerned about You or Your loved one might choose a facility that “ignores You” and that You might end up getting bed sores and be “ignored”. Let me be clear about the treatment at this facility. You will never be ignored, nor will You develop bed sores. The staff here checks You constantly to make sure that You are clean and well cared for. I have some wounds that I had before moving into this facility that had nothing to do with this facility, but this facility takes very good care of me and has “wound” doctors that documented everything about any wounds or sores and informs You about what they are doing to get You cured and the staff ALWAYS makes sure that You are clean from #1 or #2 many times a day and do not wait for You to call them, but check You as part of their routine rounds. I think this is pretty good because some people are just too shy to call and tell someone that they are “dirty”. The staff here is very good and serious about taking care of You, and have entertainment activities and games activities, so if You get bored, that is on You, not the facility. Again, I must state that the isolated incident that I posted is not a fair assessment of the facility and that I would HIGHLY recommend this facility for excellent care if You are looking for a place.

The therapy was excellent. Especially Brian he cares about you and your wellbeing. Nicole and the rest of the team helped me reached my goals to go home. Highly recommend these therapist because they care. They make you work. Bring your own food because the dietary doesn’t know how to cook. The admissions team and social worker are also great.

The therapy team is outstanding, they consistently get their patients up, moving, and progressing toward their goals. Mrs. Cheyenne, the social worker, is incredibly helpful and supportive. Mrs. Nicole, the Director of Rehabilitation, is highly involved in every patient’s plan of care and demonstrates genuine compassion and leadership. Diana, Melanie, Anthony, Emily, Brian, and Erin make up an exceptional therapy team. Truly a caring and dedicated staff all around!
