A star for the facility/building- it is very nice and clean except for that one time that the entire 2nd floor smells like sewage. The nurses are great personalities wise. However they could have been more observant to the patient health. I feel like to my husband's case they were not knowledgeable enough to what he is going through. (My husband have COPD, and was in creekside because he is needing rehab for his broken hip) overtime in his stay he developed congestive heart failure which he have to be admitted back to the hospital. The room he was in was not comfortable at all for him. We have reported and tried to plea to hope that something could be done. He did not have any sleep, which resulted to him to always be tired to participate to any therapy he have scheduled the next day. And nothing was done. He was instead given melatonin. (It still did not help). I have to bring some of my husband's medicine, (i was told from the beginning that medicine will be provided) one in particular is needing to be refrigerated, as I hand over the packet I verbally said that this needs to be refrigerated and it also states on the packet. By the time that I have to collect his things I was informed that the medicine was not stored in the refrigerator. And the biggest let down of all is that some personal items were stolen. Including cash, earbuds, gift card, and a nail file kit. Understandable these maybe small items. However I put my trust to this facility, the trust that they would take care of my husband, his personal being and belongings. The facility is also a permanent resident to some yet per my conversation with the person in charge he said " they cant be responsible for everything" This is a Very disappointing facility.

My daughters and I really struggled with taking my mom away from her home and moving her to Memory Care. It was a hard decision but I have found that it was absolutely the RIGHT decision! The care my mom is receiving is beyond my expectations. The staff is beyond amazing and they treat her as if she was their own family member. Where she wasn't at home, she is thriving in her new environment. I couldn't be more pleased. I would HIGHLY recommend this facility if you are needing memory care for your loved one.

I can’t say enough about the staff and care at Creekside. They were so kind and caring with my elderly father-in-law, they did everything to keep him clean, dressed and happy during his stay. Once he went under hospice care, they were with us constantly. Every request we made was answered and they went above and beyond to make us feel loved. To the nursing staff on the second floor, please know you made the last few months of his life easier and sweeter. I am so thankful to all of you. The family of Roger Horn
