This is not a good place to put your family members in unless you don't care. From Brad Scott down, there is nothing but unprofessionalism. These are the problems we encountered with this facility: 1. attempts to overcharge (I have phone calls and emails) 2. missing clothing that went to laundry and never came back when it had already been established that family does laundry . A sign was in the room (with names in clothes and receipts of purchase). Brad Scott offered to charge them to the credit card and then you don't get a response when you leave information for him to replace the clothes. 3. E coli Infection from lack of perineal care. 4. Med errors with dosages (example patient is ordered 7.5 mg of a medication, but is given 30mg tabs for 3 dosages before they realized it and order 7.5 mg) . When i was notified about the medication being given, the nurse stated that since the doctor order the medication, she assumed it was okay and didn't check. I have both doses of medication where the 30mg was given. 5. When you don't use their pharmacy (and yes they were notified) and you bring your own meds 8/7/2025 from a Medicare approved pharmacy, this facility will continue to order from their pharmacy which is being charged to Medicare/Medicaid for medications they already have. 6. when you report problems to the administrator and all they want to know is if there is audio on the camera you have in the family members room. By problems I mean... staff inserting IV's without gloves (I have pictures) and staff wiping patients from front to back with poop on the wipe while being watched on camera (e coli infection), staff not keeping your family clean (i.e. no oral care/face care in the mornings, patient is also allowed to sit in urine/feces until the change of shift). 7. Poor communication from top to bottom. If you inform them that you family member has an appointment or is being picked up, they will not pass the information on and when you get there you get the element of surprise that your family member has been in bed all day and still has a gown on. After the second time reporting the issue, I realized they just didn't care. 8. When you take you family member out and come back after the 6 the door is locked and the nursing staff at the desk will look at you as you ring the doorbell until they are ready to push the button that is laying on the desk next to them. This has taken 10-20 minutes at times for them to let you in. 9. The staff has no problems sitting a tray of food next to a patient who can't reach it. There will be a breakfast and lunch tray that hasn't been touched and then the staff will bring in a dinner tray. 10. Unprofessional behavior for adults. When you complain, you are the problem. The staff will then will start ignoring you and again this goes from the top with Brad Scott down. 11. I notified them that we were removing our family member from this facility for all these problems then this happened on 26SEP25: I got there after work and my mother was still in the bed with a gown on, there was no discharge paperwork, her tv was still on the wall (even though I had spoken with the maintenance person 2 days prior and asked him to remove it on the morning of her discharge and was told that it would be completed), I took it down myself, and the medication had not been prepared for her release. Not to mention the social worker refused to give report to the receiving facility because we transported the patient. 12. We had to seek outside medical help and now have to see a specialist for a problem noticed by the facility in APR2025, but was not taken care of. By this time I am not surprised by anything not completed within this facility. If you are not monitoring your statements, you should. If you are not involved in your family members care, you should be. When you start doing this you will become the next victim , but at least you will know where you family member stands in the grand scheme of things and find another facility that will take better care of your loved ones.
To whom it may concern; My wife and I visited a friend of ours that I used to work with since the mid 1980’s and is now residing at the Heritage Nursing & Rehabilitation Center. I had called a day before our visit and asked if they had a certain protocol we had to adhere to while visiting and the receptionist was very polite, knowledgeable, friendly while providing informative answers to make our visit effortless. She is very helpful and customer oriented as were all staff members we met during our very pleasant visit with our friend. All your staff members and employees are very friendly and people oriented and these qualities make them the perfect employees for this family members caring facility. The facility is very tidy and very well kept by the staff on hand. They seem to take a lot of pride on their work and assisting the family members that are living in the facility. The whole staff are very friendly, knowledgeable, courteous and helpful to all of us visiting our friends or family members in this facility. KUDOS to ALL your staff, they are all keepers 👍🏼💕‼️. This place has a very unprofessional management team. Only bring your enemies here. The CNA’s are worth their weight in gold. Special shout out to Alexis, Debra, Nee Nee, Candy, Twin and Alvin. Also, Ms. Tiffany. The rehab team was great but I wasted 4 weeks of rehab. The management team kept me stressed instead of focusing on my recovery. Jessica the social worker claims all of the decisions were made by “Laura” well my husband or I never met or talked to “Laura” Also, the food is horrible, how do a rehab center run out of diaper, chucks and wipes? What are you doing with the patient money???? I had to purchase my own mattress because theirs were horrible.
